ABOUT STRONGER TOGETHER
Stronger Together is CANI's chronic disease prevention and community health program, crafted in partnership with the Lake County Health Department through the Health First Indiana grant.
We meet Lake County residents where they are — in schools, community centers, health fairs, and faith communities — delivering evidence-based education, biometric screenings, and wellness resources that can make a real difference. This program is for adults, students, and expectant families across Lake County, Indiana.
We believe where chronic
disease is preventable,
not inevitable.
WHY WE DO WHAT WE DO
Chronic disease doesn't have to be a life sentence. In Lake County, more than 66% of adults are overweight or obese — and the burden of heart disease, diabetes, and hypertension falls hardest on communities that already face the most barriers.
At CANI, we believe that access to education, screenings, and support can change that trajectory. Through the Stronger Together program, we've reached 334+ Lake County residents, delivered 242 biometric screenings, hosted 22 community events, and built partnerships with 10+ organizations across the Region.
We don't just talk about health equity. We show up for it.
